This study evaluated a “free-running” cardiac MRI method for pediatric patients with congenital heart disease, using ferumoxytol contrast. The technique is fully self-gated, meaning it does not require ECG triggering, respiratory navigation, breath-holds, or prospective scan adjustments. Eighteen children underwent 5D whole-heart coronary magnetic resonance angiography on a 1.5T scanner, with images reconstructed to resolve both cardiac and respiratory motion.<br /><br />The protocol was successful in all patients and took under 8 minutes to acquire. Coronary artery origins were visible in 90% of evaluated cases, and image quality was similar in intubated/anesthetized patients and those breathing freely. Respiratory motion was well resolved across four breathing states, and coronary vessel sharpness was comparable between groups. The only notable difference was that the right coronary artery was visualized over a slightly longer segment in intubated patients.<br /><br />A key finding was that ECG signals were often degraded by the MRI acquisition itself: half of the ECG traces were corrupted, while the self-gating signals remained reliable in all patients. The study also showed that temporal resolution could be flexibly adjusted after acquisition, reaching as fine as 25 ms in children with higher heart rates.<br /><br />Overall, the authors conclude that this push-button 5D MRI approach provides high-resolution whole-heart coronary imaging in pediatric CHD patients, with excellent anatomical detail and simplified workflow. It may reduce reliance on anesthesia, intubation, and complex scan planning, making coronary CMR more practical and patient-friendly in clinical settings.
