A 20-year-old woman with chest pain and a large pericardial effusion was found on CT and cardiac MRI to have a well-circumscribed enhancing pericardial mass with pericardial thickening and septations. PET-CT showed hypermetabolism in the mass, pericardium, and mediastinal nodes, suggesting malignancy. Biopsy confirmed epithelioid malignant pericardial mesothelioma, a rare and aggressive tumor. She underwent surgical resection and adjuvant cisplatin-pemetrexed chemotherapy. The case highlights that pericardial masses are uncommon, and cardiac MRI is the most useful imaging test for diagnosis and characterization, with PET-CT important for staging.
