A 58-year-old woman presented with a week of non-radiating chest pain, elevated troponin, and inflammatory markers, but normal coronary angiography. Echocardiography and cardiac MRI revealed two infiltrative right-sided cardiac masses involving the right ventricle, septum, and pericardium. Endomyocardial biopsy confirmed diffuse large B-cell lymphoma. She was transferred for chemotherapy and completed six cycles, with marked reduction of the mass on follow-up echocardiography. At 10 months, she achieved full clinical and laboratory remission. The case highlights the importance of advanced cardiac imaging when chest pain and cardiac biomarkers suggest myocardial injury despite normal coronaries.
