A previously healthy 9-year-old boy developed elevated troponin and abnormal ECG after being run over by a trailer. Echocardiography and cardiac MRI showed a true left ventricular aneurysm, not a pseudoaneurysm. He was initially managed conservatively, but six weeks later returned with palpitations, a 22-beat run of ventricular tachycardia, and rapid aneurysm enlargement. He underwent successful aneurysmectomy and patch repair with a Gore-Tex patch. Pathology confirmed a true aneurysm. One year later, he remained clinically well with good systolic function and no recurrent arrhythmias. Cardiac MRI was key for diagnosis and surgical planning.
