Video Summary
A 34-year-old man from Africa was evaluated for fever, abdominal pain, and weight loss. An abdominal CT incidentally found a small hypodense lesion in the left ventricular lateral wall. Cardiac MRI showed an intramyocardial cyst with fluid-like T1/T2 characteristics, no perfusion, and no late gadolinium enhancement. PET-FDG revealed hypermetabolism in the same area, raising suspicion for an isolated cardiac hydatid cyst. Serology was negative, likely due to the small cyst size. The patient started albendazole, with stable findings on follow-up MRI. Surgery was deferred pending further PET-FDG reassessment and because of the thin left ventricular wall.
