Video Summary
This case describes a man with chest pain, fever, weight loss, and elevated LDH who was found on CCTA and CMR to have a large infiltrative mass involving the right heart and pericardium. Key imaging clues suggested primary cardiac lymphoma: a bulky pericardial/right-sided mass, low perfusion, heterogeneous T1/T2 signal, patchy enhancement, and most importantly, encasement of the right coronary artery without invasion. Biopsy confirmed CD20-positive lymphoma. PET showed intense uptake, and chemotherapy led to complete remission. Primary cardiac lymphoma is rare, and CMR is especially helpful for recognizing its characteristic tissue and vascular patterns.
