Practice guidelines strongly influence cardiac MRI (CMR) adoption, but high-level evidence from randomized trials is limited. Of 1,390 CMR-related studies on ClinicalTrials.gov, only 15 (about 1%) test CMR as an intervention. Most current CMR abstracts and studies are nonrandomized or observational. The speaker highlighted CE-MARC 2, a multicenter trial comparing CMR, SPECT, CT, and invasive angiography in suspected angina, with unnecessary angiography as the main outcome. CMR strategy trials could expand its role in ischemic heart disease, arrhythmias, heart failure, cardio-oncology, and hypertrophic cardiomyopathy.
