Recent CMR trials in stable coronary disease suggest imaging can safely reduce unnecessary angiography and revascularization. In CMARC2, a CMR-guided strategy lowered unnecessary caths compared with UK NICE guidelines, with no increase in 12-month MACE. In MR-INFORM, CMR was non-inferior to invasive angiography with FFR for guiding management, again with low and similar event rates, but fewer revascularizations. Together, these studies support CMR as an effective gatekeeper and treatment guide. Future work should compare anatomical vs functional tests, clarify CT, stress echo, FFR-CT, PET, and explore optimal serial testing and hybrid imaging strategies.
