This multicenter study evaluated automated retrospective valve tracking in flow MRI to quantify regurgitation and forward flow across four heart valves. Six international sites analyzed 20 subjects each, using varied scanners and protocols, while a central reader repeated the measurements. The study compared inter-observer and inter-site agreement using Bland-Altman analysis, ICC, and coefficient of variation. Results showed strong agreement for net forward volume, regurgitation fraction, and net forward variation. Despite technical differences between centers, measurements were clinically acceptable in over 75% of cases, with a target threshold of under 10% variation.
