Late gadolinium enhancement (LGE) MRI is a gold-standard technique for assessing myocardial viability, infarction, and scar patterns in both ischemic and non-ischemic cardiomyopathy. It works because abnormal myocardium has a larger interstitial space, causing gadolinium to be retained longer and appear bright on T1-weighted images. Imaging is performed about 10 minutes after contrast, using a TI-scout to null normal myocardium. Difficult cases may require phase-sensitive inversion recovery, free-breathing methods, or motion correction. Proper parameter optimization is essential to reduce artifacts and improve scar detection.
