Video Summary
Treadmill stress CMR combines exercise testing with cardiac MRI to capture real-world physiologic data that pharmacologic stress cannot, including symptoms, ECG changes, arrhythmias, heart rate, blood pressure, and functional capacity. It is performed using an MR-compatible treadmill positioned near the scanner to allow peak-stress imaging within 60–90 seconds. The protocol includes rest imaging, Bruce treadmill exercise, rapid post-stress perfusion and cine imaging, then recovery and late gadolinium enhancement. Studies show it is safe, feasible, prognostic, and diagnostically strong, with better wall visualization than echo and higher accuracy than SPECT.
